I havent seen that link before, and I dont think there is one for bupe, a conversion chart I mean.
because most of the time it SEEMS like doctors start with a 16mg dose. this topic of "standard" dosing, has been discussed here alot.
Most doctors want you to be over the ‘ceiling dose’ so that you dont have the ‘ups and downs’ of other opiates. Meaning, if you over the ceiling dose, it carries MOST people over 24hours……eliminating the need for more than once a day dosing. Unless your being dosed for pain too, then its just complicated 
Im taking it for both, so Im perscribed 24mg. But I usually get away with 16 just fine.
